Claim for lost or damaged item

Claim forms are submitted by the Owning Library, or the Dropsite (if the Owning Library is not a Dropsite).
Please fill in the information requested. For Dropsite names, use the name shown on the courier website, not OCLC symbols or other forms of the name.
When you submit the form, the information will be sent directly to Senvoy, and a copy to the Orbis Cascade Alliance.

Step 2. After you Submit the form, you'll see a Thank You page that includes the information you submitted. Please print TWO copies, one for your records and one to enclose with the INVOICE or documentation showing cost (price) of item.
